About The Position

Evaluates, examines, diagnoses, and treats patients in collaboration with their supervising and/or substitute supervising physician according to established guidelines and protocols. Maintains and promotes the well-being of the patient through preventative healthcare and guidance. May provide care in both the hospital and outpatient setting. May include weekends and shift work. Includes surgical first assist responsibilities.

Requirements

Graduate of accredited Nursing & Nurse Practitioner Program.
Master's Degree in Nursing.
Current licensure to practice as a Registered Nurse in the State of Pennsylvania.
Current Certification as CRNP.
Current Certification for Prescriptive Authority with DEA license.
PA Act 153 Clearances (Act 34-PA Criminal Record Check, Act 33-PA Child Abuse History, Act 73-FBI Fingerprint screening within 2 weeks of hire).
BLS Certification.
Certified Midwife.
Related training/experience (preferably within the past five years).
One-year medical/surgical experience.

Responsibilities

Manages the physical and psychological problems of the patient, including identifying normal and abnormal findings, implementing and reassessing individualized treatment plans, evaluating responses to medications and treatments, and prescribing immunizations.
Communicates effectively with physicians and other healthcare providers, maintains problem/medication lists, ensures accurate and timely documentation, assesses patients, facilitates consultations, directs patients to appropriate care levels, and assists in identifying resources for treatment or rehabilitation.
Demonstrates leadership by solving problems independently, delegating patient care appropriately, maintaining accountability, functioning as a resource person, acting as a patient advocate, preceptor/mentoring new hires/students, assisting in cost reduction, and completing annual competency requirements.
Provides for patient safety in compliance with hospital and physician practices policies, adhering to infection control processes, utilizing two forms of patient identification, practicing National Patient Safety Goals, and using LEAN principles.
Assists in the department's compliance with all regulatory agencies, participates in continuous survey readiness, the non-punitive medication reporting system, ensures accountability for narcotics, and participates in office emergency drills.
Performs procedures, assists in surgery, and provides care to the surgical patient, including positioning, prepping, assisting the surgeon, maintaining sterile techniques, performing skin closures, monitoring patients, performing suture removal and dressing changes, and being available for emergency surgery call.
Directs and provides appropriate Pre and Post-operative care, including performing history and physicals, providing preoperative teaching, performing patient rounds, ordering appropriate testing, documenting assessments and progress, communicating with the supervising physician, preparing discharge planning, and dictating discharge summaries.
Performs clinical responsibilities to maximize patient care and outcomes, assumes appropriate code team roles, facilitates timely treatment of abnormal findings, develops workflow patterns for patient transfers/discharges, and participates in outcome management.
Interviews, evaluates, and assesses patients in the clinic setting, provides recommendations for treatment, adjusts medication, and provides patient/family education regarding medications.
Collaborates with the supervising physician and treatment team to achieve desired outcomes and ensure continuous patient care flow, alerting the team to status changes, reviewing treatment goals, and communicating pertinent information.
Documents in the medical record, including patient's mental status, screenings, assessments, medication changes, prescriptions, patient complaints, telephone contact, session times, and signs/dates all documentation.
Participates in Performance Improvement activities to enhance quality of care, customer service, or practice efficiencies, identifies initiatives to improve patient outcomes and satisfaction, assists in chart review/audits, and supports implemented initiatives.
