About The Position

Clover Health is changing how care is delivered for people with complex health needs. Our In-Home Care team brings compassionate, high-quality primary care directly into members’ homes. Supported by care teams and Clover Assistant, our data-driven platform, you’ll deliver proactive, relationship-based care where it matters most. We’re looking for a Physician Assistant in Atlanta who wants to manage complex care, build real patient relationships, and make a measurable impact.

Requirements

  • Certified Physician Assistant, PA-C.
  • Located in or near Atlanta, Georgia.
  • Able to travel locally for in-home visits.
  • At least 1 year of PA experience in a clinical setting.
  • Active DEA registration.
  • Current Basic Life Support certification.
  • Comfortable managing chronic illness and medically complex patients.
  • Confident working with autonomy while collaborating with physicians and care teams.
  • Compassionate, grounded, and able to build trust quickly with patients and families.
  • Organized, responsive, and comfortable using technology to support care delivery.

Nice To Haves

  • Home care, house call, geriatrics, complex care, or value-based care experience is a plus.

Responsibilities

  • Deliver longitudinal primary care to Clover members in their homes.
  • Manage a dedicated panel of patients with chronic, complex, and advanced medical needs.
  • Build trusted relationships with members, caregivers, and families.
  • Assess clinical needs, create care plans, and support patients through ongoing care.
  • Partner with physicians, care teams, and community providers to improve outcomes.
  • Use Clover Assistant and clinical data to guide proactive, personalized care.
  • Help prevent avoidable emergency room visits and hospitalizations.
  • Participate in a shared on-call rotation to ensure members have continuous support.
