About The Position
This position is for a Physical Therapist (PT) in a Skilled Nursing Facility (SNF) working day shifts from 8:00 AM – 4:30 PM, with 35 guaranteed hours per week for a 13-week contract. The PT provides rehabilitation services focused on improving patient mobility, strength, balance, and overall functional independence.
Requirements
- Active Physical Therapist license
- Graduated from an accredited Physical Therapy program (DPT or PT)
- Strong clinical assessment skills
- Strong treatment planning skills
- Strong documentation skills

- Experience in SNF, rehabilitation, or long-term care settings
Responsibilities
- Perform patient evaluations
- Develop individualized care plans
- Deliver therapy interventions including gait training, strengthening exercises, balance training, and mobility improvement
- Monitor patient progress
- Modify treatment plans
- Document therapy sessions
- Collaborate with interdisciplinary teams to ensure optimal recovery and safe discharge planning
