A SNF (Skilled Nursing Facility) unit provides short-term, inpatient rehabilitation for patients who no longer require hospital care but still need daily skilled nursing and therapy services. The primary goal is to restore strength, mobility, and independence, enabling patients to safely return home or transition to a less intensive care setting. These units operate with an interdisciplinary team under physician supervision. Typical patients are recovering from conditions such as stroke, joint replacement surgery, fractures, cardiac issues, or general deconditioning. Physical therapists play a crucial role in assessing patient mobility, developing individualized treatment plans, providing therapeutic interventions like strengthening exercises, balance training, and gait training, managing pain, and educating patients on the safe use of assistive devices. They also contribute to discharge planning, including assessing home safety needs. Patient stays typically range from 2 to 4 weeks, with therapy sessions averaging 1 to 2 hours per day. Coverage is often partially provided by Medicare Part A when specific criteria are met.
