Physical Therapy Assistant - 10 to 15 Visits Per Week
About The Position
Home Health Care, Inc. is seeking a licensed Physical Therapy Assistant to provide in-home mobility expertise. This role involves implementing personalized treatment plans developed by a licensed Physical Therapist to help patients regain strength, range of motion, and independence. The Physical Therapy Assistant will deliver hands-on care, including therapeutic exercises, mobility training, gait education, and adaptive strategies. Responsibilities also include documenting interventions in EMR, educating patients and caregivers on home exercise routines and fall prevention, and collaborating with the supervising PT and interdisciplinary care team.
Requirements
- Licensed Physical Therapy Assistant in the state of Minnesota
- Valid driver’s license, reliable vehicle, and current insurance
- Strong communication skills, with the ability to educate and motivate patients

Responsibilities
