About The Position

Requirements

Licensed Physical Therapy Assistant in the state of Minnesota
Valid driver’s license, reliable vehicle, and current insurance
Strong communication skills, with the ability to educate and motivate patients

Responsibilities

Implement personalized treatment plans developed by a licensed Physical Therapist, helping patients regain strength, range of motion, and independence
Deliver hands-on care including therapeutic exercises, mobility training, gait education, and adaptive strategies to support recovery
Document all interventions and patient responses in EMR to track clinical progress
Educate patients and caregivers on home exercise routines and fall prevention strategies, empowering them with confidence
Collaborate regularly with the supervising PT and interdisciplinary care team to ensure holistic patient care
