About The Position

At Country Meadows, our therapy teams help residents move with confidence, independence, and purpose. Physical Therapist Assistants (PTAs) play an essential role in delivering resident-centered physical therapy services that support rehabilitation, functional independence, and quality of life. Under the direction and supervision of a licensed Physical Therapist, the PTA works collaboratively with residents, families, caregivers, and the interdisciplinary care team to implement individualized treatment plans in accordance with Pennsylvania licensure requirements, professional standards, and organizational policies.

Requirements

  • Graduate of an accredited Physical Therapist Assistant program
  • Current licensure or eligibility for licensure as a Physical Therapist Assistant in the Commonwealth of Pennsylvania
  • Knowledge of physical therapy treatment techniques appropriate to the PTA scope of practice
  • Ability to implement treatment plans under the supervision of a licensed Physical Therapist
  • Ability to work collaboratively within an interdisciplinary care team
  • Must be able to communicate the English language effectively both orally and in writing, and comprehend the same

Responsibilities

  • Implement individualized physical therapy treatment plans under the supervision of a licensed Physical Therapist and in accordance with established plans of care
  • Provide therapeutic interventions including therapeutic exercise, functional and mobility training, gait training, manual therapy as directed, and application of physical agent and mechanical modalities
  • Observe patient responses to treatment and communicate progress, tolerance, and concerns to the supervising Physical Therapist
  • Assist the Physical Therapist during evaluations, testing, and complex procedures as directed
  • Instruct patients, families, and caregivers in safe mobility techniques and therapeutic activities under supervision
  • Organize and facilitate individual and group treatment activities in collaboration with the Physical Therapist
  • Participate in patient care conferences, rehabilitation meetings, and interdisciplinary collaboration
  • Ensure adherence to patient schedules, departmental expectations, and rehabilitation service standards
  • Maintain accurate, timely clinical documentation in compliance with regulatory, payer, and organizational requirements
  • Support a safe care environment by identifying and reporting safety concerns and equipment needs
  • Deliver care in a respectful, compassionate, and resident-centered manner that recognizes individual needs and rights
