About The Position

One Home Health is seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our growing home health team on a part-time basis. This role offers the opportunity to provide individualized care to patients in their homes while working alongside an experienced and supportive clinical team.

Requirements

An active Physical Therapist Assistant license in the state of employment.
Graduation from an accredited PTA program recognized by the American Physical Therapy Association (APTA).
At least six months of experience working as a Physical Therapist Assistant.
Strong communication, documentation, and interpersonal skills.
A valid driver's license, dependable vehicle, and current auto insurance meeting agency and state requirements.
The ability to travel to patient homes within the designated service area.

Responsibilities

Deliver physical therapy treatments and educational support to patients in their homes based on the established physician-approved care plan.
Complete required visit documentation accurately and submit records within agency timelines and compliance standards.
Observe and report patient responses, progress, concerns, or scheduling changes to the supervising Physical Therapist and care team.
Work collaboratively with nurses, therapists, and other healthcare professionals to support coordinated patient care.
Participate in interdisciplinary meetings, case reviews, and care planning discussions as needed.
Follow all safety procedures, infection prevention practices, and equipment guidelines during patient visits.
Safeguard patient privacy and maintain confidentiality in accordance with applicable regulations.
Adhere to agency policies and all state, federal, and professional practice requirements.
Attend staff development programs, training sessions, and quality improvement initiatives.
Perform other related responsibilities as assigned.
