Physical Therapy Assistant- PACE program - No Nights, weekends or holidays
About The Position
Fallon Health's Summit ElderCare® is a Program of All-Inclusive Care for the Elderly (PACE) for individuals 55 and older who wish to continue living safely at home. This role involves providing and documenting physical therapy treatments under the supervision of a Physical Therapist, supervising supportive personnel, and participating in educational services. The position also involves acting as a clinical instructor and teaching inter-department continuing education. Requirements
- Associates degree from an accredited Physical Therapy Assistant Program.
- Must possess a valid driver's license.
- Must attest to no disqualifiers per Driver Safety Policy.
- Must possess and provide proof of minimal state required auto insurance.
- Must have reliable transportation.
- Licensure in Massachusetts or licensure pending via examination or reciprocity.
- CPR certification or willingness to be certified within 60 days of hire.
- Minimum of 1 year experience with elderly population.
Responsibilities
- Under the supervision of a Physical Therapist, performs processes involved in the delivery of age-appropriate patient care.
- Provides care appropriate to the patient population served.
- Completes all required competency assessments.
- Identifies, reports, and documents the need for modification of treatment techniques.
- Determines, reports, and documents possible causes of change in patient’s physical/mental status, tolerance to treatment, and unanticipated rate of progress, and determines appropriate modifications.
- Identifies, reports, and documents acute changes in patient physiological status and communicates these to the Physical Therapist.
- Modifies treatment as appropriate within the plan of care.
- Implements appropriate treatment techniques as specified in the therapist plan of care and all treatment procedures as necessary for the specific patient population under the direction of the Physical Therapist.
- Utilizes all appropriate equipment per department protocols and standards.
- Communicates effectively with patients and caregivers.
- Provides and documents specific training related to the patient’s dysfunction.
- Provides patients with educational materials appropriate to patient and family needs.
- Provides clinical instructions to affiliate physical therapist assistant students and accepts assigned students for designated clinical affiliation experiences.
- Actively cooperates with other personnel to achieve clinical education program objectives.
- Provides clinical education/instruction of staff, actively participating in the department’s clinical education program serving as an instructor for staff continuing education in-services.
- Meets department standards for scheduling and productivity.
