About The Position

Fairview Range is looking for a Physical Therapy Assistant to join our Hibbing Home Care and Hospice team. We’re offering an up to $15,000 sign-on bonus! Minimum amount of $5,000 for a one-year commitment. We consider it an honor to provide care for our patients and their families and serve communities across the Iron Range. The RN Case Manager coordinates the delivery of all the care/services appropriate to the needs of the patients of all ages and their families using an interdisciplinary team approach. An individualized plan of care with specific goals is developed for each patient in collaboration with the physician. The physical therapy assistant is responsible for providing direct patient care in the home setting based on a treatment plan established by the Physical Therapist. Requirements

One year acute, clinic, or long-term care experience.
Graduate of an APTA accredited Physical Therapist Assistant program.

Nice To Haves

One year of home care experience.

Responsibilities

Implement client treatment plan as instructed by a physical therapist.
Effectively uses the treatment plan to maintain and/or restore strength, endurance, coordination, relaxation and range of motion, promote healing, relieve pain and improve clients' functional level.
Teach the clients, families, or other health care provider to perform selected treatment procedures and functional activities.
Communicate any client care issues or changes in client status to the Physical Therapist and/or Nurse Case Manager promptly.
Maintain complete, accurate and timely clinical records/documentation on all clients according to agency policy.
