Physical Therapist Assistant (PTA) – Home Health (PRN to Part-Time) Location: Fort Collins, Colorado Are you passionate about helping patients live their healthiest, most independent lives? Requirements

Graduate of an accredited Physical Therapist Assistant program (CAPTE, APTA, CAHEA, or AMA approved)
Active Physical Therapy Assistant (PTA) license in the state of practice (Required).
CPR Certification
A strong desire to serve, support, and improve the quality of life for homebound patients.

Nice To Haves

1+ years of physical therapy experience in a home health setting (Preferred).

Responsibilities

Administer physician-prescribed physical therapy treatments to help patients rebuild strength, improve mobility, and manage physical disabilities.
Empower individuals to achieve their highest level of physical capability, productivity, and independence.
Partner with interdisciplinary home care team members to develop and adapt individualized patient care plans.
