About The Position

Physical Therapist assignment in a Skilled Nursing Facility (SNF) setting for a 13-week contract with a consistent day shift schedule from 8:00 AM to 4:30 PM. This role offers 35 guaranteed hours per week and focuses on delivering rehabilitative care to patients in a post-acute environment. The PT will work to improve patient mobility, strength, balance, and overall functional independence through individualized treatment plans. The position emphasizes patient-centered care and collaboration with interdisciplinary teams to support recovery and enhance quality of life.

Requirements

• Minimum 2 years of Physical Therapist experience
• Active State PT License required
• BLS certification required
• Strong knowledge of therapeutic exercises, mobility training, and functional rehabilitation
• Ability to manage a caseload efficiently
• Excellent communication and interpersonal skills
• Ability to collaborate effectively with interdisciplinary teams

Nice To Haves

• Experience in SNF or rehab setting preferred
