Requirements

• Current Physical Therapist license in the State of Arizona
• Graduate of an accredited Physical Therapy program
• Current CPR certification
• Valid driver’s license, reliable transportation, and automobile insurance that meet agency requirements
• Strong clinical judgment, organization, and communication skills
• Ability to work independently while collaborating effectively with an interdisciplinary team
• Commitment to compassionate, patient-centered care
• Compassionate, professional, and patient-focused
• Organized and dependable
• Comfortable providing care independently in patients’ homes
• Skilled at communicating with patients, families, and members of the care team
• Committed to helping patients achieve meaningful functional goals
