About The Position
A Physical Therapist (PT) is needed for a Full-Time Home Care position in Cleveland, OH. Enjoy a great work-life balance with a growing Home Care Company. This full-time PT will cover Start of Care (SOC) and ongoing therapy visits across different areas, including the west side of Cleveland and Mentor.
Requirements
- Current state license to practice as a Physical Therapist in Ohio.
- Graduate of an accredited Physical Therapy program.
- Current CPR certification.
- Valid driver's license, reliable transportation, and auto insurance.
- Strong communication, organizational, and time-management skills.
Nice To Haves
- Prior home care experience preferred.
Responsibilities
- Complete Start of Care (SOC) evaluations and ongoing therapy visits.
- Develop treatment plans by identifying a problem list and methods to achieve identified goals.
- Coordinate treatment plan with the patient, family, physician, and other team members.
- Adapt treatment to meet the individual needs of the patient.
- Review the quality and appropriateness of the total services delivered.
- Supervise PTAs and HHAs.
- Complete clinical documentation in a timely manner.
