About The Position

The Physical Therapist - Value Based Care position involves assessing patient conditions, establishing treatment goals, and developing care plans in collaboration with physicians, patients, and their families. The role ensures appropriate interdisciplinary treatment, consults with physicians on physical therapy's impact on patient health, and meticulously documents findings in the medical record. Responsibilities include educating patients and families on care and exercises, serving as an information resource, and supervising Physical Therapy Assistants and Technicians. The therapist also acts as a resource for and provides ongoing education to Physical Therapy staff, assists the director with patient care issues, and maintains patient confidentiality. The job holder must demonstrate current competencies for the position.

Requirements

  • Requires a Valid and active State Licensure
  • Two years of experience in an out-patient or remote physical therapy clinic.
  • Experience with orthopedics is required.
  • Current state license.
  • Knowledge of physical therapy principles, standards, and applications.
  • Knowledge of clinic policies, procedures, regulations, safety/infection control, quality control.
  • Knowledge of how to use physical therapy equipment.
  • Knowledge of medical terminology.
  • Skill in appropriate evaluation and treatment of patients.
  • Skill in effectively communicating with in-house as well as external physicians and referral sources.
  • Skill in using a computer system/EMR to accurately reproduce all appropriate documentation.
  • Skill in accurate recognition of patient/clinic danger and appropriate fall risk patients.
  • Ability to establish/maintain effective working relationships with patients, providers, and staff.
  • Ability to supervise and mentor physical therapy staff.
  • Ability to communicate clearly in verbal and written form.
  • Ability to be a resource to patients and staff.

Nice To Haves

  • Clinical Doctorate (DPT) preferred.
  • Experience in remote work preferred.

Responsibilities

  • Assesses patient condition, establishes goals, treatments, and care plans in conjunction with the physician, patient/family.
  • Ensures that appropriate treatment is performed in an interdisciplinary fashion, consulting with physicians about the impact of physical therapy factors on the patient’s health status.
  • Documents findings, treatments, and other patient information for the medical record.
  • Educates patient/family on appropriate care and use of various therapies/exercises.
  • Serves as information/education resource for patients, families, and staff.
  • Supervises Physical Therapy Assistants and Physical Therapy Technicians.
  • Serves as a resource to Physical Therapy staff.
  • Provides ongoing education to the Physical Therapy staff.
  • Assists the director with patient care issues.
  • Maintains patient confidentiality.
  • Demonstrates current competencies for the job position.
