About The Position

Oak Crest Village by Erickson Senior Living is hiring a resident-centered Physical Therapist to provide home care to residents living on its campus. The role supports residents throughout their continuum of care and addresses their physical, spiritual, emotional, and clinical needs through person-centered care. The position is a flexible, PRN arrangement with daytime hours and pays $55.00 per hour. Candidates should have experience in Home Health or Home Care.

Requirements

• Degree and training must meet current Medicare and state requirements.
• Valid and current state Physical Therapy license.
• Experience and ability to complete documentation to meet reimbursement and regulatory requirements.
• Experience with the OASIS documentation system.
• Experience in Home Health or Home Care fields.

Nice To Haves

• Therapeutic experience with older adults across multiple diagnostic groups.

Responsibilities

• Provide holistic, person-centered care that supports residents’ physical, spiritual, emotional, and clinical needs.
• Implement residents’ personal care plans, focusing on their strengths, preferences, and preferred routines while offering comfort, companionship, and belonging.
• Work with the interdisciplinary clinical team to ensure residents’ needs are addressed throughout their care.
• Complete documentation that meets reimbursement and regulatory requirements.
