About The Position

Oak Crest Village by Erickson Senior Living is hiring a resident-centered Physical Therapist to provide home care to residents living on its continuing care campus. The role supports residents’ daily living and care needs through person-centered, holistic care and collaboration with the interdisciplinary clinical team. Candidates must have experience in Home Health or Home Care. The position is a flexible, PRN arrangement with daytime hours and pays $55.00 per hour.

Requirements

• Degree and training must meet current Medicare and state requirements.
• Valid and current state Physical Therapy license.
• Experience and ability to complete documentation to meet reimbursement and regulatory requirements.
• Experience in Home Health or Home Care fields.
• Experience with the OASIS documentation system.

Nice To Haves

• Therapeutic experience with older adults across multiple diagnostic groups.

Responsibilities

• Provide holistic support for residents’ physical, spiritual, emotional, and clinical needs.
• Implement residents’ personal care plans with attention to their strengths, preferences, and routines, promoting comfort, companionship, and belonging.
• Work with the interdisciplinary clinical team to address residents’ needs throughout their care.
• Complete documentation to meet reimbursement and regulatory requirements.
