About The Position
Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout Richmond, Henrico, Chesterfield, Mechanicsville, Glen Allen, Midlothian, Hanover, and surrounding communities. This is a PRN opportunity offering flexibility, autonomy, and the ability to make a meaningful impact every day. As a Home Health Physical Therapist, you'll help patients regain strength, restore mobility, improve balance, and rebuild confidence in their own homes. You'll have the time to focus on each patient, create personalized treatment plans, and celebrate progress.
Requirements
- Active Virginia Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Organizational Skills
Nice To Haves
- Home Health Experience Preferred but Not Required
- Experience with OASIS is a plus
Responsibilities
- Complete evaluations, reassessments, and discharge visits
- Develop individualized plans of care based on patient goals
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on mobility, therapeutic exercises, fall prevention, and home safety
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians and interdisciplinary team members
- Complete timely and accurate clinical documentation
