About The Position
BAYADA Home Health Care is seeking an experienced Physical Therapist for a full-time outpatient position serving older adults who live in Assisted Living and Independent Living communities in Shrewsbury. The role focuses on evidence-based interventions that optimize clients’ function and independence and help them age in place. New graduate Physical Therapists are encouraged to apply, and prior experience is not required. The Physical Therapist works as part of a multidisciplinary healthcare team, providing outpatient visits within designated senior living communities. The position includes assessing clients, developing and revising individualized plans of care, delivering therapy interventions, educating clients and those supporting them, and documenting care using a touch pad tablet. Base pay is $48.00–$54.00 per hour, depending on qualifications. BAYADA offers a comprehensive benefits package. Requirements
- Current Massachusetts Physical Therapist license.

Responsibilities
- Make outpatient visits to clients in designated senior living communities.
- Perform diagnostic tests and measurements, including assessments of joint mobility and range of motion, transfer status, stability, gait patterns and appearance, muscle strength and endurance, balance, and safety.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance work, and other interventions.
- Continually assess and revise the Physical Therapy plan of care.
- Participate in multidisciplinary care plan meetings, including meetings with community staff as appropriate.
- Educate and instruct clients, family members, and community staff about therapy services and activities that promote health, safety, and independent living.
- Document observations, interventions, and evaluations related to client care management and services provided using a touch pad tablet.
