About The Position

BAYADA Home Health Care is seeking an experienced Physical Therapist for a full-time or part-time opportunity providing outpatient visits in senior living communities in Brewster. The therapist will treat geriatric residents of assisted living and independent living communities, using proactive, evidence-based interventions to optimize function and independence and support clients in aging in place. New graduates are encouraged to apply, and prior outpatient experience is not required. The role is part of a multidisciplinary health care team.

Requirements

• Current Massachusetts Physical Therapist license.

• Outpatient experience is a plus but is not required.

Responsibilities

• Make outpatient visits to clients in designated senior living communities.
• Perform diagnostic tests and measurements, including assessment of joint mobility and range of motion, transfer status, ambulation patterns and appearance, muscle strength and endurance, balance, and safety.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance activities, and other interventions.
• Continually assess and revise the Physical Therapy plan of care, and participate in multidisciplinary care plan meetings with community staff as appropriate.
• Educate and instruct clients, family members, or community staff about therapy services and activities that promote health, safety, and independent living.
• Document observations, interventions, and evaluations related to client care management and services provided using a touch pad tablet.
