About The Position

The Physical Therapist works in a skilled nursing setting, evaluating and treating patients recovering from orthopedic surgeries, neurological events, cardiac conditions, and general deconditioning. The therapist develops individualized treatment plans and collaborates with the interdisciplinary care team—including nursing, physicians, and families—to help patients achieve functional outcomes and return to their highest level of independence. The position offers a manageable caseload and respect for clinical judgment. Structured mentorship is available for new graduates.

Requirements

• Doctor of Physical Therapy (DPT) or a Master's degree from an accredited physical therapy program.
• Current New York State physical therapist license or eligibility for one.
• Passion for geriatric rehabilitation and functional outcomes.
• Strong communication and teamwork skills.

Responsibilities
