About The Position
Fountain Inn Post Acute is a 66-bed skilled nursing facility located just 20 minutes from downtown Greenville, South Carolina. With new leadership and positive changes underway, we are building a workplace where staff feel heard, supported, and valued. Our smaller setting allows you to truly connect with your residents and coworkers, creating a meaningful and team-oriented environment. As a Physical Therapist (PT), you will evaluate and treat residents to improve mobility, strength, and functional independence. You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, individualized care.
Requirements
- Degree in Physical Therapy (Bachelor’s required; Master’s or Doctorate preferred)
- Active, unencumbered Physical Therapist license in South Carolina (required)
- Must maintain all continuing education and remain in good standing with the State Board
- Strong clinical, communication, and organizational skills
Nice To Haves
- Experience in a long-term care or rehab setting preferred
- Familiarity with PointClickCare (PCC) and Casamba preferred
Responsibilities
- Perform patient evaluations and develop individualized treatment plans
- Provide therapy to improve strength, mobility, balance, and overall function
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely clinical documentation
- Communicate with physicians, residents, families, and care team members
- Participate in care planning and discharge planning
- Conduct staff education and in-service training
