About The Position
Evergreen is a premier Life Plan Community that offers independent living, assisted living, memory care, long term care, short term rehab and in home care. As a non-profit Life Plan Community led by Christian values we have served older adults ages 55 and better in the Oshkosh community since 1967. We are dedicated to providing the best resident centered care. We are seeking a skilled and motivated Physical Therapist (PT) to join our dynamic rehab team. In this role, you will assess, plan, and implement individualized therapy programs for both short-term rehab patients and long-term care residents. The ideal candidate is a caring professional who enjoys working in a team environment and is committed to helping individuals regain or maintain their independence in daily living.
Requirements
- Bachelor�s or Master�s degree in Physical Therapy from an accredited program
- Current state licensure as an Physical Therapist (PT)
- Excellent communication, organization, and time management skills
- Compassionate, patient-centered mindset with a passion for helping others
Responsibilities
- Evaluate patients� functional abilities and develop individualized treatment plans
- Provide hands-on therapy to improve residents� and patients� abilities
- Document progress and adjust treatment plans as needed
- Educate patients, residents, and caregivers on exercises and strategies to improve function
- Collaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workers
- Ensure compliance with regulatory requirements and documentation standards
- Maintain accurate records using electronic medical record (EMR) systems
