About The Position
Helping Hand Nurse, LLC is a home health care agency seeking Physical Therapists for our home health patients. We are actively hiring in multiple locations across Philadelphia and surrounding County. Clinicians choose their own schedule, benefit from quick and easy documentation, and have support staff available to assist them.
Requirements
- Current unrestricted PT license
- CPR certified
- Knowledge of computerized documentation and office software required
- Clean background and criminal record
Nice To Haves
- Minimum one (1) year of home care experience preferred.
- Three (3) to five (5) years of clinical experience.
- Previous supervisory experience.
- Experience with OASIS (although this can be trained)
Responsibilities
- Visiting patients in their homes and providing skilled home health care clinical visits.
- Providing care for individuals based on a personalized Plan of Care.
- Providing physical therapy services to patients according to a written physician's plan of care.
- Assessing and evaluating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care, as well as evaluating the home environment.
- Assessing for muscle strength, mobility, gait, ROM--potential for rehab.
- Directing physical therapy treatment.
- Instructing patients and families/caregivers in the use and care of therapeutic appliances.
- Determining priority needs for physical therapy.
- Reporting to the physician patient's reaction to treatment or changes in condition.
