About The Position
Prime Healthcare Home Health is seeking a dedicated and skilled Physical Therapist to join our team. This full-time position involves providing comprehensive home health care services to patients, establishing care plans, and administering treatments. The role requires effective communication with physicians and staff, supervision of Home Health Aides, and active participation in department functions.
Requirements
- Bachelor, Masters, or Doctorate Degree in Physical Therapy from an accredited program.
- One year recent clinical experience.
- Current license as a Physical Therapist.
- Current BLS certification.
- Current driver’s license, car insurance, car registration, and reliable automobile.
Nice To Haves
- Home Care experience preferred.
Responsibilities
- Performs comprehensive assessments, evaluations and re-evaluations on patients to establish plan of care and goals.
- Performs patient treatment according to Physical Therapy procedures of modalities, therapeutic exercise, gait training and other accepted methods.
- Communicates pertinent information verbally or written to physician, other staff members, patients/caregivers.
- Supervises and directs Home Health Aide’s care in relationship to the patient’s rehab program.
- Demonstrates active participation in home care department functions.
