About The Position

Central Coast Home Health is a locally owned, fast-growing agency with an excellent reputation across the Central Coast. We are seeking Home Health Physical Therapists (PT) to provide high-quality, patient-centered care in multiple San Luis Obispo Areas: North County (Atascadero/Templeton/Paso Robles), South County (Pismo Beach/Arroyo Grande/Nipomo/Santa Maria), Santa Maria-Lompoc, and Lompoc-Santa Ynez Valley. This role offers flexibility, independence, and strong earning potential in a supportive team environment.

Requirements

• Active California Physical Therapist (PT) license
• CPR certification
• Valid driver’s license + clean driving record
• Reliable transportation & auto insurance
• Ability to work independently

Responsibilities

• Provide in-home physical therapy services
• Develop individualized care plans
• Collaborate with interdisciplinary team members
• Document patient progress accurately and timely
