This full-time position in Outpatient Rehabilitation requires a Physical Therapist to interpret physician referrals, conduct comprehensive evaluations (musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary), and establish individualized treatment plans based on evaluation results and functional goals. The role involves implementing appropriate techniques, monitoring patient response, adjusting treatment plans, and providing status updates to physicians and other healthcare professionals. Responsibilities also include identifying factors affecting patient comfort, planning and completing patient discharge (including home programs, teaching, referrals, and equipment ordering), and collaborating with stakeholders for optimal patient care. The position requires meticulous documentation following department policies and accreditation standards, including evaluations, progress notes, team conference reports, and discharge summaries. Daily statistics, billing, and scheduling are also part of the role, along with applying insurance guidelines for authorizations and visit numbers. The Physical Therapist must maintain productivity standards, adhere to billing and documentation policies, and ensure the treatment area is clean and orderly. The role involves performing various manual techniques, patient positioning, transferring, and transporting duties, which require lifting and pushing/pulling with proper technique. Skill in using modalities, exercise equipment, and other devices, along with knowledge of their indications and contraindications, is essential. Promoting patient safety through participation in safety initiatives and adherence to safety and infection control measures is crucial. The position also involves participating in performance improvement activities, managing an assigned caseload, and assisting with coverage needs. Professional growth is encouraged through continuing education, department in-services, and professional literature. The role may include orienting new staff, participating in co-worker evaluations, and supervising physical therapy assistants, extenders, and students, with specific responsibilities for delegation, direction, and supervision while maintaining overall responsibility for the patient's plan of care.
