About The Position

Focused Post Acute Care Partners (FPACP) is seeking a Licensed Physical Therapist to join their team. FPACP is a dynamic company focused on providing excellent long-term care. They have a recognition program called ROCKStar (Recognizing Outstanding Care and Kindness) which offers a gamified experience for team members to level up and potentially attend an annual gala. The Physical Therapist will evaluate patients, develop treatment plans in collaboration with physicians, and provide direct physical therapy treatments. This role also involves instructing patients and caregivers, supervising Physical Therapy Assistants, and utilizing various therapeutic modalities for pain management and muscle/nerve function stimulation.

Requirements

Bachelor's Degree, Master's Degree, or Doctorate Degree in Physical Therapy from an accredited college or university.
Current license/registration by state of hire as a Physical Therapist.

Responsibilities

Evaluate patients and develop physical therapy treatment programs in collaboration with the physician.
Provide physical therapy treatments to patients.
Administer complex active and passive therapeutic exercises, therapeutic massages and various modalities.
Instruct, motivate, and assist patients/caregivers in learning and improving activities of daily living.
Supervise and direct the efforts of the Physical Therapy Assistant, if one is assigned.
Establish, implement, and evaluate treatment programs, plan of care, goals, and discharge plans for patients as related to Physical Therapy needs and in conjunction with the physician.
Revise plans and goals based on changes in patient status.
Provide direct patient care services to ensure the effectiveness of Physical Therapy programs.
Apply heat, light, water, and electrically based treatments/modalities (i.e., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy, etc.) to stimulate muscle/nerve function and/or for pain management, as prescribed, with proven competency and available equipment.
Prioritize, plan, treatment and travel times to provide for maximum efficiency, yet meet the needs of the patients.
Ensure orders are carried out in a professional manner satisfying professional ethics, patient rights, and State and Federal codes and regulations.
Facilitate effective communication amongst other clinicians providing care to the patient.
Assist in coordinating the Physical Therapy program with other appropriate health care providers/ disciplines providing services to the patient.
Participate in interdisciplinary patient care conferences as applicable and maintain ongoing communication with all disciplines involved in the patient Plan of Care.
