About The Position

Focused Post Acute Care Partners (FPACP) is seeking a Licensed Occupational Therapist to join their team at Focused Care at Linden in Linden, TX. FPACP emphasizes a culture of excellence and recognition through its ROCKStar program. This role involves evaluating patients, developing and implementing occupational therapy treatment programs in collaboration with physicians, and providing direct patient care. The position also includes instructing patients, supervising an Occupational Therapy Assistant, and ensuring effective communication and coordination with other healthcare providers.

Requirements

Bachelor's Degree, Master's Degree, or Doctorate Degree in Occupational Therapy from an accredited college or university.
Current license/registration by state of hire as an Occupational Therapist.

Responsibilities

Evaluate patients and develop occupational therapy treatment programs in collaboration with the physician.
Provide occupational therapy treatments to patients.
Administer complex active and passive therapeutic exercise, ADL retraining, and various cognitive techniques.
Instruct, motivate, and assist patients in learning and improving activities of daily living.
Supervise and direct the efforts of the Occupational Therapy Assistant, if one is assigned.
Establish and continuously evaluate and implement treatment programs, plans of care, goals, and discharge plans for patients related to Occupational Therapy needs in conjunction with the physician.
Revise plans and goals based on changes in patient status.
Provide direct patient care services to ensure the effectiveness of Occupational Therapy programs.
Apply heat, light, water, and electrically based treatments/modalities (e.g., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy) to stimulate muscle/nerve function and/or for pain management, as prescribed, with proven competency and available equipment.
Prioritize, plan, treat, and manage travel times to provide for maximum efficiency while meeting patient needs.
Ensure orders are carried out in a professional manner satisfying professional ethics, patient rights, and State and Federal codes and regulations.
Facilitate effective communication among other clinicians providing care to the patient.
Assist in coordinating the Occupational Therapy program with other appropriate health care providers/disciplines providing services to the patient.
Participate in interdisciplinary patient care conferences as applicable and maintain ongoing communication with all disciplines involved in the Plan of Care.
