About The Position

LifeSpan Therapy is seeking a Physical Therapist to help residents recover mobility, reduce pain, and improve physical function following injury, surgery, or illness. The therapist assesses residents’ physical conditions, develops personalized treatment plans, and provides therapeutic interventions to promote healing and quality of life. The role involves collaboration with residents, families, and multidisciplinary healthcare teams, monitoring resident progress, adjusting treatment as needed, and educating residents about exercises and lifestyle modifications to support long-term health and independence. The position may occasionally require travel to other locations to provide physical therapy coverage.

Requirements

• Licensed Physical Therapist (PT) and graduate of an ACOTE-accredited school.
• Current state licensure to practice physical therapy.
• Valid Indiana Operator's license.
• Demonstrated experience in resident care, including assessment and treatment planning.
• Basic computer literacy for documentation and communication.

Nice To Haves

• Experience working in outpatient or rehabilitation settings.
• Strong interpersonal and communication skills to engage effectively with diverse patient populations.
• Continuing education or advanced training in infection control practices.

Responsibilities

• Ensure Physical Therapy Services facilitate optimal resident outcomes and comply with MDS procedures.
• Assess facility residents for physical therapy needs at admission or when indicated, and provide appropriate services.
• Coordinate admission to and discharge from Physical Therapy with the Interdisciplinary Team, Restorative Care Nurse, and staff nurses.
• Develop and implement individualized treatment plans based on resident needs and clinical best practices.
• Provide therapy or assign the resident to a Physical Therapy Assistant (PTA), and communicate treatment-plan information to nursing and other personnel to support a team approach.
• Monitor resident progress and accurately document treatment outcomes in compliance with healthcare regulations.
• Educate residents and their families on injury prevention, health maintenance, and therapy techniques.
• Use basic computer skills to manage resident records, schedule appointments, and communicate with the care team.
• Occasionally travel to other locations to provide physical therapy coverage when needed.
