Requirements

Graduate of an accredited Physical Therapy program
Current California Physical Therapist license in good standing
Current CPR certification preferred
Strong clinical assessment and treatment planning skills
Excellent communication and interpersonal abilities
Ability to work effectively in a team-oriented healthcare environment
Commitment to providing exceptional resident care and outcomes

Nice To Haves

Skilled nursing facility experience preferred but not required

Responsibilities

Evaluate residents and develop individualized treatment plans based on clinical findings and physician orders
Provide skilled physical therapy services to residents in accordance with established plans of care
Assess and address mobility limitations, balance deficits, strength impairments, pain management, and fall risk reduction
Establish realistic, measurable treatment goals and modify plans as needed
Supervise and collaborate with Physical Therapist Assistants (PTAs) and rehabilitation staff
Complete timely and accurate documentation in compliance with facility, state, and federal requirements
Communicate resident progress and recommendations with physicians, nursing staff, families, and the interdisciplinary team
Educate residents and caregivers on exercises, safety techniques, and discharge recommendations
Ensure compliance with all professional, ethical, and regulatory standards
