About The Position

Signature Healthcare at Home is hiring a full-time Physical Therapist to serve the Northeast Portland Metropolitan Area, including neighborhoods such as Alberta, Hollywood, Parkrose, Laurelhurst, Gateway, and surrounding communities. Requirements

• Current Oregon Physical Therapist license (or ability to obtain).
• One year of PT experience preferred; home-health experience a plus.
• Strong communication, assessment, and clinical reasoning skills.
• Ability to work independently in a field-based environment.
• Reliable transportation and valid driver’s license.

Responsibilities

• Conduct home-based physical therapy evaluations.
• Develop individualized treatment plans focused on functional mobility and independence.
• Deliver therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
• Perform home-safety assessments and recommend appropriate DME.
• Manage a field-based caseload with efficiency and autonomy.
• Collaborate with interdisciplinary team members to ensure coordinated care.
• Maintain accurate documentation in accordance with regulatory standards.
• Support safe transitions from hospitals, SNFs, and outpatient settings.
