About The Position
Signature Healthcare at Home is hiring a full-time Physical Therapist to serve the Northeast Portland Metropolitan Area. This role is ideal for PTs who thrive in dynamic, community-based care environments. Requirements
- Current Oregon Physical Therapist license (or ability to obtain).
- One year of PT experience preferred; home-health experience a plus.
- Strong communication, assessment, and clinical reasoning skills.
- Ability to work independently in a field-based environment.
- Reliable transportation and valid driver’s license.

Responsibilities
- Conduct home-based physical therapy evaluations.
- Develop individualized treatment plans focused on functional mobility and independence.
- Deliver therapeutic interventions including gait training, strengthening, balance work, and home-exercise program development.
- Perform home-safety assessments and recommend appropriate DME.
- Manage a field-based caseload with efficiency and autonomy.
- Collaborate with interdisciplinary team members to ensure coordinated care.
- Maintain accurate documentation in accordance with regulatory standards.
- Support safe transitions from hospitals, SNFs, and outpatient settings.
