About The Position
CommuniCare Rehab, a member of the CommuniCare Family of Companies, has a PRN opening for a Physical Therapist (PT) at Bridgewater Healthcare Center. In this role, therapists are In-House Team Members and have the opportunity to make a direct impact with residents through therapy. CommuniCare is a rapidly growing company with 140 centers across multiple states.
Requirements
- A Bachelor's, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
- Hold a valid or pending State Physical Therapy License.
Responsibilities
- Evaluate, review, and screen patient’s/resident’s medical charts and evaluations under physician order.
- Develop a treatment plan in accordance with patient’s needs.
- Implement treatment plan through direct treatment, as well as education of the plan of care to all team members.
- Provide necessary supervision for PTAs.
- Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of the rehab team.
- Conduct staff/family training and education when appropriate.
- Serve a wide variety of patient needs by working at multiple centers.
