About The Position

BAYADA Home Health Care is seeking a compassionate Physical Therapist (PT) to provide one-on-one, in-home care throughout Bergen County. This per diem opportunity offers flexible visits, allowing you to control your hours without minimums or pressure. You will provide meaningful work by seeing patients in their homes, performing evaluations, developing personalized treatment plans, and delivering skilled therapy focused on patient goals and independence. The role also involves educating patients and families and completing efficient electronic documentation.

Requirements

• Licensed Physical Therapist in good standing in New Jersey
• Graduation from an accredited PT program
• Passion for patient-centered, in-home care
• Reliable transportation and willingness to travel locally

Responsibilities

• See patients in their homes throughout Bergen County
• Perform evaluations and develop personalized treatment plans
• Deliver skilled therapy focused on patient goals and independence
• Educate patients and families with care and clarity
• Complete simple, efficient electronic documentation
