Requirements

Licensed Physical Therapist
comfortable working independently
managing a mobile caseload
helping patients achieve safe functional outcomes in their home environment

Responsibilities

Provides skilled physical therapy services to patients in their homes, focusing on improving mobility, strength, balance, safety, and independence.
Performing evaluations, developing and implementing individualized treatment plans, providing therapeutic interventions, educating patients and caregivers, documenting care, and collaborating with healthcare teams to support functional recovery.
Conduct in-home PT evaluations and reassessments
Create and progress individualized treatment plans
Provide therapeutic exercises, gait training, balance training, and functional mobility training
Educate patients and caregivers on home exercise programs and safety strategies
Document visits and patient progress according to clinical standards
Coordinate care with physicians and other healthcare providers
