About The Position

Concierge Home Care is hiring a Home Health Physical Therapist (PT). This role offers more control over your schedule, meaningful patient relationships, and the ability to increase your income based on productivity. The coverage area includes Tallahassee, FL / Leon, Franklin, or Taylor Counties, with territories designed to minimize drive time through clustered scheduling. The position offers a salary+ model with earning potential of $88,000 – $95,000+ for 24 productivity points, pay-per-visit at $70+ per visit plus Medicare differential pay for visits over 24 points, quarterly bonus opportunities, and mileage reimbursement plus a data plan stipend. Consistent patient volume is provided. Therapists choose Concierge Home Care for flexibility and autonomy, allowing them to create and manage their own schedules for greater control over work-life balance. The company provides technology that works for the therapist, including AI-assisted charting to reduce documentation time and a user-friendly EMR (WellSky/Kinnser), enabling more time with patients. Requirements

• Active, unencumbered Florida Physical Therapist license
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Evaluate patients and develop individualized treatment plans
• Provide therapeutic exercises and mobility training
• Educate patients and caregivers on safety and recovery
• Collaborate with physicians and interdisciplinary teams
• Supervise Physical Therapist Assistants (PTAs)
• Complete documentation using streamlined technology
