About The Position
Sobe Innovative Rehab is seeking a licensed Physical Therapist to provide therapy services for hospice patients in a home setting. The role supports older adults in maintaining their independence and quality of life through compassionate, evidence-based care. The therapist evaluates and treats patients in an assisted living community and/or home setting, with coverage in ZIP codes 33805 and 33801. This is a PRN position: therapists work on an as-needed basis and choose the assignments they wish to accept. The role offers scheduling flexibility, competitive compensation, clinical autonomy, a supportive team environment, and transparent payment reporting.
Requirements
- Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited program.
- Active Florida Physical Therapist (PT) license.

Responsibilities
- Evaluate and treat residents in an assisted living community and/or home setting.
- Develop and implement individualized treatment plans.
- Collaborate with physicians, families, and the interdisciplinary care team.
