Requirements
- Graduate of an accredited Physical Therapy program.
- Current and active California Physical Therapist license (or eligibility for new grads).
- Ability to relate positively and effectively with residents, families, and professional colleagues.
- Strong commitment to confidentiality and resident rights.
Responsibilities
- Evaluate residents to determine physical therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders.
- Develop and implement individualized plans focused on improving mobility, strength, balance, and functional independence.
- Supervise Physical Therapy Assistants (PTAs) and assist the nursing department with the training of Restorative Aides.
- Communicate regularly with physicians, nursing staff, and families regarding progress, goals, and discharge planning.
- Complete timely and accurate clinical documentation in compliance with facility, state, and federal requirements.
- Recommend and facilitate the ordering of durable medical equipment (DME) such as walkers, canes, and splints to enhance resident independence.
- Provide in-service training for facility staff and educate families on fall prevention and home exercise programs.
