About The Position

Nightingale Home Healthcare is seeking part-time Physical Therapists (PTs) to join their dedicated team. The role involves providing skilled, compassionate therapy services to home health patients, teaching family members proper care techniques, and collaborating closely with the care team and physicians. The company has been in the home health industry for over 25 years, focusing on high-quality, one-on-one care that allows patients to thrive in their homes.

Requirements

• Active Physical Therapist (PT) license
• Minimum 1 year of PT experience (Home Health/OASIS preferred)
• Physical (within 12 months of first patient contact)
• Current TB test (within last 12 months)
• Current CPR certification
• Valid driver’s license & reliable transportation
• Must be available to attend orientation at our Willowbrook office
• Available Monday–Friday, 8 AM–5 PM

Nice To Haves

• WellSky (Kinnser) EMR experience a plus

Responsibilities

• Evaluate patients within 72 hours of receiving a physician referral
• Develop and implement individualized Plans of Care
• Provide therapy services and supervise PTAs and HHAs
• Communicate effectively with physicians and the interdisciplinary team
• Maintain accurate, timely documentation in EMR
• Evaluate treatment effectiveness and update care plans as needed
• Participate in interdisciplinary team meetings and continuing education
• Maintain patient confidentiality per HIPAA standards
