About The Position

H2 Health Prime Living is seeking a Physical Therapist to provide 1:1 patient care in senior living communities. This role offers a flexible schedule, with options for part-time or full-time employment (10-35+ visits). The position focuses on providing care within a luxury Senior Living community in Tallahassee, FL, emphasizing a supportive, clinician-led, and patient-focused environment. It is suitable for both new graduates and experienced PTs seeking a meaningful career change.

Requirements

• Must have a degree from an accredited Physical Therapy program.
• An active or eligible state license to practice as a Physical Therapist (PT).
• New graduates are welcome.
• Outpatient experience is a plus, but we value passion and a commitment to patient care.

Responsibilities

• Evaluate and treat patients with orthopedic, post-operative, neurological, and sports-related conditions in an outpatient setting.
• Design and implement personalized treatment plans aimed at restoring function and enhancing quality of life.
• Document patient progress accurately using EMR.
• Collaborate with a multidisciplinary team to deliver holistic, patient-centered care.
• Contribute to a positive, clean, and professional clinic environment that reflects H2 Health’s culture of excellence.
