About The Position
Human Touch Home Health is hiring a licensed Physical Therapist (PT) to provide home health services in Blacksburg, Christiansburg, Radford, Pulaski, and surrounding New River Valley communities. This is a full-time role that offers flexibility, autonomy, and the opportunity to build meaningful relationships with patients.
Requirements
- Active Virginia Physical Therapist license
- Valid driver’s license and reliable transportation
- Strong documentation and communication skills
Nice To Haves
- Home health experience preferred
Responsibilities
- Complete evaluations, treatment plans, reassessments, and discharge planning
- Provide skilled physical therapy services in patients’ homes
- Coordinate care with physicians and the interdisciplinary team
- Manage your own daily schedule and service area
