What You’ll Be Doing (High Level) Providing skilled PT care in patients’ homes Completing admissions and routine visits Developing and carrying out individualized care plans Collaborating with nurses, therapists, and providers Participating in interdisciplinary team meetings (The formal job description comes later—this is about the experience.)
