About The Position
Navitas Healthcare, LLC is seeking a licensed Physical Therapist to provide home health therapy services in West Portland, OR, and surrounding areas. This role is ideal for skilled PTs interested in patient-focused, in-home care with flexible scheduling in a supportive home health environment.
Requirements
- Active Oregon PT license.
- 1 year of skilled home health experience required.
- First-timers accepted.
- BCLS certification.
- Reliable transportation for home visits.
- Ability to work independently in a field-based environment.
Nice To Haves
- Oasis and EPIC experience preferred.
Responsibilities
- Provide skilled PT services in patients’ homes (up to 6 visits per day).
- Develop and implement individualized care plans to improve functional independence.
- Conduct patient assessments, monitor progress, and adjust therapy interventions as needed.
- Document all patient visits using EPIC/Oasis software.
- Maintain patient safety and adhere to infection control and home health regulations.
- Coordinate care with nurses, physicians, and other multidisciplinary team members.
