Requirements

• Current PT License in the state of CA with 1 years’ experience.
• Highly Motivated with Passion for working with the senior population and the ability to blend healthcare and hospitality service philosophies.
• Dependable, Good communication and Organizational Skills.
• Ability to work alone and as a team member.
• Active Physical Therapy License in the state of CA.
• Dependable Transportation, Current Driver’s License and auto Insurance.
• Current CPR
• Ability to perform the physical aspects of the position.
• TB testing.

Nice To Haves

• 1 year Home Health experience preferred but not necessary

Responsibilities

• Initiate diagnostic, preventive and rehabilitative procedures as appropriate to the patients care.
• Create a Plan of Care by evaluation and assessment of patient’s levels of functionality.
• Maintain continuity of and or multidisciplinary services whenever needed to ensure good coordination of care.
• Timely Documentation according to state, federal and organizational guidelines.
• Participation in Staff meetings, and In-Service Reviews.
