About The Position

This is a full-time Physical Therapist position in an outpatient pediatric setting at Cleveland Rehabilitation. The role involves evaluating patients, developing and implementing care plans, and documenting patient progress. The Physical Therapist will also communicate with physicians and other healthcare professionals, coordinate patient schedules, and provide discharge planning. Additionally, the role includes supervising Physical Therapist Assistants and Rehab Tech/Aides as required by licensure.

Requirements

  • Bachelor's degree in Physical Therapy program required.
  • Applicable state license for Physical Therapy required.
  • BLS required per policy guidelines.

Responsibilities

  • Evaluates patients, selects and administers assessment techniques, and synthesizes information to establish an appropriate patient care plan.
  • Selects and administers necessary modalities, procedures, exercise programs, therapeutic groups, and patient education.
  • Gathers and interprets appropriate data from the medical record, physician reports, patient, and/or family.
  • Establishes functional goals in conjunction with the patient and provides evidence-based intervention to work towards meeting these goals.
  • Adjusts the plan of care as needed based on ongoing assessment.
  • Documents evaluations, tests, progress notes, daily treatment notes, attendance records, patient/family education, outcomes, discharge notes, and charges.
  • Communicates patient's evaluation and subsequent progress to physician, referral sources, and other clinical staff.
  • Coordinates patient's schedule and treatment programs, and discharge planning.
  • Provides supervision of PTA's and Rehab Tech/Aide as required for licensure.
  • Demonstrates any appropriate exercise and activities to patients/caregivers.
