About The Position

Mobile Health Team Inc. is seeking an experienced Physical Therapist (PT) - Home Health for a 13-week travel assignment in Kāne‘ohe, HI. This role involves planning and conducting treatment programs to restore and improve patient functions, completing evaluations, establishing individualized treatment plans, providing therapy interventions, and supervising assistants. The position requires comprehensive and accurate clinical documentation within established deadlines.

Requirements

  • Minimum of 2 years of Physical Therapist (PT) experience.
  • Hawaii state PT license.
  • Productivity expectation of 25 points per week.
  • Bachelors degree (Masters preferred).
  • 1 year of PT home health experience.
  • Epic experience.
  • Travel experience.
  • Must have and maintain current Basic Life Support (BLS) certification during employment.
  • Effective team skills including interaction, support, and respect.
  • Strong organizational skills and ability to multitask.
  • Willingness to participate in continuing education and training programs.
  • Strong attention to detail and ability to work independently.
  • Excellent communication skills, professionalism, and reliability.
  • Ability to learn and apply basic computer skills including locating and opening documents, completing forms, opening and sending e-mail messages.
  • Must pass a drug screen and background check.
  • Completion of additional facility onboarding forms as assigned.

  • Masters degree preferred.

Responsibilities

  • Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
  • Serves as co-leader in interdisciplinary approach to patient groups.
  • Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
  • Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
  • Provides therapy interventions using appropriate treatment protocols and techniques.
  • Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
  • Completes comprehensive and accurate clinical documentation within established deadlines.
  • Supervises assigned assistants.
  • Performs other job-related duties as assigned.
