About The Position

Mobile Health Team Inc. is seeking an experienced Physical Therapist (PT) - Home Health for a 13-week travel assignment in KÄ ne‘ohe, HI. This is a contract position with a start date of 9/8/2026. The role involves planning and conducting treatment programs to restore and improve patient functions, serving as a co-leader in interdisciplinary patient groups, completing evaluations, and providing therapy interventions. The position also requires establishing individualized care and discharge plans, completing clinical documentation, supervising assistants, and performing other job-related duties.

Requirements

  • Must have 2 years PT experience
  • Must have HI state PT license
  • Productivity expectation is 25 points per week
  • Must have a Bachelors degree
  • Must have 1 year PT home health experience
  • Must have Epic experience
  • Must have Travel experience
  • Hawaii State License Requirement
  • Must have and maintain current Basic Life Support (BLS) certification during employment
  • Effective team skills including interaction, support, and respect are required.
  • Strong organizational skills and ability to multitask
  • Willingness to participate in continuing education and training programs
  • Strong attention to detail and ability to work independently
  • Excellent communication skills, professionalism, and reliability
  • Ability to learn and apply basic computer skills including locating and opening documents, completing forms, opening and sending e-mail messages.
  • Drug screen and background check (facility required)

Responsibilities

  • Plans and conducts segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
  • Serves as co-leader in interdisciplinary approach to patient groups.
  • Completes evaluations to ensure quality standards and addresses assessment areas that require attention.
  • Uses appropriate clinical techniques and tools and establishes individualized treatment plans for patients.
  • Provides therapy interventions using appropriate treatment protocols and techniques.
  • Establishes individualized care and discharge plans and ensures plans are communicated to appropriate parties.
  • Completes comprehensive and accurate clinical documentation within established deadlines.
  • Supervises assigned assistants.
  • Performs other job-related duties as assigned.
