About The Position

Human Touch Home Health has an immediate need for a Physical Therapist (PT) to provide home health visits to patients in Northwest Washington, DC. We have patients who need care now and are looking for a DC-licensed PT who can add visits to their current schedule or build a PRN caseload with our team.

Requirements

• Active Washington, DC Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Dependable transportation to patient visits
• Strong clinical and communication skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health and OASIS Experience Preferred, but Not Required
• WellSky Experience is a Plus

Responsibilities

• Complete PT evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor patient progress and update treatment plans
• Complete progress assessments and discharge planning
• Coordinate with physicians, nurses, and other members of the care team
• Complete timely documentation using WellSky
