About The Position

Human Touch Home Health is seeking Physical Therapists (PTs) for Full-Time and PRN opportunities serving patients throughout Floyd, Giles, and Pulaski Counties. This role offers independence, flexible scheduling, one-on-one patient care, and the opportunity to directly impact a patient's everyday life. The service area includes Floyd, Pearisburg, Narrows, Pembroke, Rich Creek, Pulaski, Dublin, Fairlawn, and surrounding communities. Assignments can be coordinated based on patient needs, availability, and preferred service areas.

Requirements

• Active Virginia Physical Therapist License Required
• Graduate of an accredited Physical Therapy program
• Valid Driver's License and reliable transportation
• Strong clinical, communication, and documentation skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health and OASIS experience preferred, but not required
• WellSky experience is a plus

Responsibilities

• Complete PT evaluations in patients' homes
• Develop and manage individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training
• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans
• Coordinate care with the interdisciplinary team
• Complete timely documentation using WellSky
• Manage patients from evaluation through discharge
